Scheduling Policies

What to know before booking.

  • Credit card information is required to make an appointment and is kept on file for commercially insured and private pay only. Medicaid recipients do not have to have a credit card form on file.

  • A deposit is required to reserve an initial assessment/evaluation.

  • If an initial evaluation is canceled at least 7 days in advance, the deposit is refundable.

  • If an initial evaluation is canceled with less than 7 days' notice, a new $250 deposit is required to reschedule.

  • For insured patients, applicable balances may be applied toward future co-pays.

  • A missed appointment fee for established patients is equal to the full session fee.
